“Patient Oriented, Outcome Driven”: Celebrating Case Management Week October 14-20

Care Management is celebrating National Case Management Week again this year. Our Team has Nurse Care Coordinators, Nurse Utilization Reviewers, Social Workers, payer specialists and placement specialist. We are a vital part of the interdisciplinary team at AAMC and we work with multiple post-acute providers, community resources and insurance companies as well.

As a team, we balance the needs of the patient and family with the services and resources available to them while constantly promoting quality health care for them, the provider, and the payer. A few examples of our tasks include: determining with the patient and family what their discharge needs are;  providing short term crisis intervention and connection to resources in the community; providing clinical reviews for insurance companies; and providing information for post acute caregivers to promote safe transitions in care.
