Kristina Andersen, RN, takes home Mae Muhr Award for S.M.A.R.T. poster

Kristina Andersen, RN, took home the Mae Muhr Award (which comes with $500) for her S.M.A.R.T. Discharge Protocol poster submission at the MNA Conference. Great job, Tina!

S.M.A.R.T. is an acronym for Signs, Medications, Appointments, Results, and Talk With Me.  The S.M.A.R.T. discharge protocol includes a worksheet designed to prompt patients and families to jot down questions or concerns about their discharge before they are discharged. It’s also reviewed at the time of discharge for accuracy and completeness.

We hope this process will open up conversations between patients and their families, RNs, MDs, case managers, and other health care team members prior to the day of discharge, as well as increase patient satisfaction and understanding.

Check out the S.M.A.R.T. Discharge Protocol poster at the Evidence-Based Practice Fair on Friday, Nov. 18 from 8 a.m. – 4 p.m. in the 1st floor hallways.
