Happy National Case Management Week (Oct 12-18)!
AAMC and the Care Management Department are celebrating National Case Management Week from October 12-18!
The Case Management Society of America states that “case management is based in the fact that when an individual reaches the optimum level of wellness and functional capability, everyone benefits: the individuals being served, their support systems, the health care delivery systems and the various reimbursement sources.”
We are often asked, “Just what is it that you do?”
Our department is integral in assisting patients and families with discharge planning, crisis intervention, hospice, guardianship, placement issues post hospitalization, assistance for the uninsured or underinsured, insurance reviews, and coordination of care across the continuum of hospital stay to return to the community. If a patient needs to go to a skilled nursing facility for IV antibiotics or daily physical therapy at discharge, if a family wants their loved one home with hospice, if a patient is unable to fill their prescriptions, if the physical therapist recommends home health physical therapy, if they need a substance abuse referral, the Care Management team assists patients and families with all these issues and more for a safe discharge from the hospital and a successful return to the community.
At AAMC, we have both Nurse Case Managers and Social Workers. We have support staff of Payor Specialists who assist with insurance reviews and Support Central Coordinators who assist with skilled nursing facility placement and transportation.
This year we have continued training to combine the Care Coordinator Nurse and Utilization Review Nurse roles to a Nurse Case Manager. The Transitional Care Nurse has joined our department. We also added three Nurse Clinical Documentation Improvement Specialists who review the medical record concurrently and interact with the physician/provider in real time to improve the specificity of documentation to best represent a complete quality medical record.
