Palliative care now available as inpatient consult
Chesapeake Palliative Medicine (CPM) service is now available as an inpatient consult service. Palliative care is patient- and family-centered care that optimizes quality of life by anticipating, preventing and treating suffering.
Palliative care exists throughout the continuum of illness and involves addressing physical, intellectual, emotional, social and spiritual needs to facilitate patient autonomy, access to information and choice. Patients to be referred include those with complex symptoms, declining status despite treatment, unclear goals, confusing code status and complex family dynamics. Palliative care practitioners, in collaboration with the interdisciplinary team, can help control difficult symptoms, help establish/negotiate realistic goals of care, communicate prognosis, facilitate challenging family meetings teams and assist in coordinating treatment.
CPM team is in-house Monday through Friday from 0800 to 1630 and Saturday and Sunday 0900 to 1300. A physician order can be placed for a “Palliative Consult” into EPIC. You can expect a consult to occur within twenty-four hours. Their office is located in shared space on 4 South and the intake coordinator may be reached on X 5663 or spectralink 4571.
